How to Use the Candidate Disclosure of Financial Interests Form
These forms can be completed by hand, on your computer, or on-line. BY HAND: (1) Print out a blank form from your computer; (2) fill in the blanks of the form by hand or by typewriter; and (3) sign and date the form. ON YOUR COMPUTER: (1) Fill in the blanks of the form on your computer; (2) print out the completed form; and (3) sign and date the form. ON-LINE: (1) Fill in the blanks of the form on your computer; (2) type your name, date the form, and check the “Certification” box; and (3) Use the “Submit Form” button on the form or attach the PDF form to an e-mail to electronically submit the form (click on one of the forms below to see instructions on how to submit the form electronically).
