How to Use Special Sessions Expenditures Forms
These forms can be completed by hand, on your computer, or on-line. BY HAND: (1) Print out a blank form from your computer; (2) fill in the blanks of the form by hand or by typewriter; and (3) sign and date the form. ON YOUR COMPUTER: (1) Fill in the blanks of the form on your computer; (2) print out the completed form; and (3) sign and date the form. ON-LINE: (1) Fill in the blanks of the form on your computer; (2) print name, date the form, and check the “Certification” box; (3) Attach the PDF to an E-Mail (email@example.com).
