To be filed by: 1. Initial filers. 2. All Filers in even-numbered years. 3. Filers in odd-numbered years who have more than 10 amendments or changes to the information reported for the preceding disclosure period.

How to complete the form: The form is a fillable PDF form that you can complete and save on your computer. You can also print out the form and complete it by hand. Follow the Instructions for Disclosure of Financial Interests: Long Form (Form D-201) when filling out the form.

How to file the form: To file your completed financial disclosure form, you must (1) electronically submit the form to the Hawaii State Ethics Commission (follow the instructions below), or (2) deliver or mail the paper form to the Hawaii State Ethics Commission, 1001 Bishop Street, American Savings Bank Tower, Suite 970, Honolulu, Hawaii 96813. If you file a paper form, remember to sign and date your form.

How to submit the form electronically: To submit your form electronically, you must complete the fillable PDF form on your computer.

After you have entered all the required information on your form, type your name and the date on the bottom of the last page and check the “Certification” box. SAVE your completed PDF form to your computer.

Now use your e-mail program to electronically submit your financial disclosure form to the Commission as explained below.

If you are using a dedicated e-mail program (such as Outlook or Lotus Notes):

  1. Click “SUBMIT FORM” on the bottom of the last page of the form. This will cause an e-mail window to open automatically with your completed PDF form attached. Do not type any additional information in the subject line or body of the e mail and do not include any additional attachments with your form.
  2. Click “SEND.”
  3. You can e-mail additional attachments to the Commission at ethics@hawaiiethics.org, or mail the attachments to the Hawaii State Ethics Commission, 1001 Bishop Street, American Savings Bank Tower, Suite 970, Honolulu, Hawaii 96813.

If you are using a non-dedicated e-mail program (such as AOL Mail, Gmail, Windows Live Mail or Yahoo Mail):

  1. Open your web-browser e-mail program and create a new e-mail message.
  2. Type or copy and paste the following e-mail address in the “To” field of your e-mail message:

Receipt Confirmation: You will receive a confirmation e-mail from the Hawaii State Ethics Commission when your filing is received. This may take several minutes after you have sent your form.
