Financial Disclosure Short Form (D-103A)

2017

State Officials who filed last year may use this “short form” financial disclosure form (Form D-103A) to update their financial information and report any changes to their financial interests.

 

All newly appointed State Officials must file a “long form” financial disclosure statement (Form D-201). If you have more than 10 changes to report, you must also submit a new long form.  Click here for the  Long Form (D-201).
